Admissions Process

SELECTING DESTINY AS YOUR SERVICE PROVIDER

SERIVICE ELIGIBILITY Registry of Unmet Needs

Destiny Management is contracted with Partners Behavioral Health Management and Vaya Health to bring NC Innovations Waiver services to children and adults with intellectual and developmental disabilities living in Burke, Caldwell, Catawba and McDowell Counties. If you are not sure if your loved one qualifies for services, please contact the Managed Care Organization in your area to  determine eligibility.

 

Applying to the Registry of Unmet Needs
The Registry of Unmet Needs is a waiting list for a spot (also known as a slot) on the NC Innovations Waiver, which covers services for people with intellectual or developmental disabilities.

Who is eligible for the Registry of Unmet Needs?

Individuals who:

  • Have a documented diagnosis of an intellectual disability or a significant developmental disability.
  • Have substantial limitations in at least three of six major life activity areas (self-care, understanding and using language, learning, mobility, self-direction, capacity for independent living).
  • Need waiver services in order to continue living in the community or to move out of an institutional setting such as an Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID) Group Home, State Developmental Center or Adult Care Home.

ADMISSIONS PROCESS

Admissions Team Review

Families who are considering Destiny Management as a Service provider will be asked to provide the agency with documentation to help our admissions team prepare for services. Our Intake Specialist will review with the Legally Responsible Person (LRP) what documentation is needed for the admissions panel to review at the time of referral.  These documents can be presented in person, faxed, emailed or requested to be sent from Care Managers directly to the agency. The Admissions Committee includes our Quality Management Coordinator, our Registered Nurse, our Intake Specialist and Interim Qualified Professional. Once our Admissions Committee has met, additional information may be requested prior to moving forward with the admission process. Our Intake Specialist will facilitate communication with families in order to complete the admissions process, including arranging for a time to sign legal and medical consents, obtain legal guardianship papers, develop short range goals, and determine a target start date to begin services.

Most current copy of Individual Service Plan, inclusive of Risk Assessment and Crisis Plan

Current copy of Psychological Evaluation

SIS Assessment (Support Intensity Scale)

Behavior Support Plan, as applicable

Medical notes and current medications

Any information on Protective Devices, or any medical devices for the consumer such as AFO’s, wheelchair, helmet, bath chair, gait belt, etc.
