Our Services

OVERVIEW OF OUR SERVICES

Destiny Management, Inc. is a provider of Innovations Waiver Services which is Medicaid funded. We serve individuals referred to the agency who have a primary diagnosis of an intellectual developmental disability. Additionally, we provide In-Lieu of Medicaid services. Services are provided based on authorized hours in an individual’s Plan. Scheduling of services is provided based on the schedule identified in the Plan. Payment for services is billed through Medicaid. We primarily serve Burke and Catawba counties. Destiny Management is endorsed to provide services in the catchment area of Partners Behavioral Health Management MCO and Vaya Health MCO.

DAY SUPPORTS

FACILITY BASED OPTIONS

Destiny Management has two adult Day Programs where Individuals with IDD can continue learning and growing and making new friends.  This service provides assistance  to acquire, retrain, or improve self-help skill, increase socialization and adaptive skills in a non-residential setting. This service can be received in the facility or community.

 BRIGHT FUTURES DAY PROGRAM

Bright Futures is a licensed day program in Hickory providing structured programming for adults with I/DD living in Catawba County.

HERITAGE DAY PROGRAM

Heritage Day is a licensed day program in Drexel providing structured programming for adults with I/DD living in Burke County.

Day Program Hours of Operation

8:30 AM-3:30 PM/Monday-Friday

DAY SUPPORTS

NON-INNOVATIONS WAIVER

Day Supports /In-Lieu of Medicaid

Long-Term Community Supports-Level 1

Recipients of this service live at home and participate in a Day Activity (Day Program and or/Community Activity) for a minimum of 1 day a week and a minimum of 3 hours per day to maintain/develop skills of active treatment as outlined in the consumers Individual Service Plan. LTCS Level 1 does not include services/support in the home and is intended for Day Activities only.

COMMUNITY NETWORKING

This service provides support to individuals with intel-lectual developmental disabilities by providing indi-vidualized day activities that support the individual in their definition of a meaningful day in a community setting with persons who are not disabled. These services are provided in the community.

VOLUNTEERING

“The clients are invaluable to Classroom Connections. Without their help, Classroom Connections would not be so organized, we would have less money to buy school supplies, and probably would not remain open for free teacher shopping…the volunteers have also been especially helpful when I’ve needed help to load and unload supplies at a moments notice from various community sites. The volunteers of Destiny Management are a wonderful asset and do volumes of things at Classroom Connections that are not mentioned.”

“It has been a pleasure working with Destiny Management over the last two years and supporting their volunteers at New Dimensions. The volunteers have been a great support for our school and have worked in many areas of our facility. The volunteers have especially been helpful during the summer months as we have prepared the school for the return of our students in September. SUPPORTED EMPLOYMENT

This service is provided in the community to individuals with intellectual developmen-tal disabilities who are 16 years old or older who need assistance with choosing, ac-quiring, and maintaining a job if they do not work in com-petitive employment.

COMMUNITY LIFE

COMMUNITY NAVIGATOR

This service provides support to individuals with intellec-tual developmental disabilities to develop social networks and connections within their local communities.

COMMUNITY TRANSITION

This service provides initial set-up expenses for adults to facilitate their transition from a Developmental Center (institution), community ICF-IID Group Home, nursing facility or another licensed living arrangement (group home, foster home, Psychiatric Residential Treatment Facility, alternative family living arrangement), or a family home / one person AFL(Alternative Family Living) to a living arrangement where the individual is directly re-sponsible for his or her own living expenses.

SUPPORTED LIVING

This service provides a flexible partnership that enables an individual /s with intellectual developmental disabili-ties to live in their own home with support from an agency that pro-vides individualized assistance in a home that is under the control and responsibility of the individual/s.

RESIDENTIAL SUPPORTS

ALTERNATIVE FAMILY LIVING

This service provides individuals with intellectual developmental disabilities assistance with acquisition, retention, or improvement in skills related to activities of daily living such as personal grooming and cleanliness, bed- making, household chores, eating, food preparation, and social and adaptive skills to enable the individual to live in the community. It is a 24 hour service provided in a home separate from the home the individual lives. These homes are known as Alternative  Family Living (AFL) homes. During the day, individuals attend a day program, job, or volunteer in their communities.

RESIDENTIAL SUPPORTS

MEDICAID IN LIEU OF LONG TERM COMMUNITY SUPPORTS LEVEL 3

Companion Living (paid roommate or alternative family) and Day Supports or Supported Employment up to 6 hours a day with different staff.

 

RESPITE

This service provides periodic relief to the family or a primary care giver principally responsible for the care and supervision of an individual with intellectual developmental disabilities residing in the community. Services can be provided in the individual’s home or in an out-of-home setting. Services can be provided during the day and night.

COMMUNITY LIVING AND SUPPORTS

This service is an individualized service that enables individuals with intellectual developmental disabilities to live successfully in their own homes, the homes of family or natural supports and to be active members of their communities. Services are provided in the home and community and can be provided daily.
